Recognizing and Treating Nail Fungus (Onychomycosis) in a Guideline-Compliant, Effective, and Sustainable Manner

 

Nail fungus (medically: onychomycosis / pathogen: dermatophytes Trichophyton rubrum) is a common, usually slow-progressing fungal infection of the fingernails and toenails. Typical symptoms include discoloration (whitish/yellowish/brownish), thickening, crumbling nail mass, ridges, and separation of the nail from the nail bed. If left untreated, the infection often persists for years and becomes a source of reinfection.

To ensure that you treat effectively and precisely, rather than "on suspicion," we follow the current German guidelines.


Why take nail fungus seriously?

Nail fungus is not just a cosmetic issue. Depending on its severity, it can

  • cause pain when walking (pressure in the shoe),
  • permanently damage the nail plate,
  • increase the risk of skin cracks and secondary infections,
  • be more problematic for people with diabetes, circulatory disorders, or immunosuppression.

Crucially: Nail fungus is treatable, but therapy requires time and a consistent approach.


Guidelines: What is currently recommended?

The authoritative guideline in Germany is the S1 guideline "Onychomycosis" (AWMF Register No. 013-003, Version 5.1, as of 05/2022).

The guideline emphasizes three basic principles:

  1. Secure the diagnosis before treating (mycological pathogen detection).
  2. Choose therapy according to severity and pattern of infection (local vs. systemic).
  3. For more severe infections, aim for a combination of systemic and topical therapy to increase the chances of success.

 

Step 1: Before treatment, the correct diagnosis


Many nail changes look "fungal" but are something else (e.g., nail psoriasis, trauma, eczema, aging dystrophy).

Therefore, the guideline recommends mycological diagnostics (nail material) using microscopy, culture, and/or PCR.

Practical tip: The better the material (correctly taken from the active area), the more reliable the result and the more targeted the therapy.

Step 2: Current treatment strategies.

 1) Topical therapy (lacquer/solution), for mild to moderate cases

 Local therapy is guideline-compliant if the infection is limited (e.g., early stages, few nails, no pronounced matrix involvement).

Typical active ingredients/approaches:

  1. Antifungal nail polishes (e.g., amorolfine- or ciclopirox-based, depending on the product/prescription situation)
  2. Regular application over many months/years (nail must grow out healthy)Why so long? Toenails
  3. grow slowly. Visible "normalization" only occurs when enough healthy nail has grown out, which often takes 9–12 months (sometimes longer).

Important for success: The thicker/more fissured the nail, the less well a lacquer penetrates. This is where podiatry comes into play.

2) Podiatric support, the underestimated success factor

In line with guidelines and extremely relevant in practice is mechanical reduction: shortening, thinning, professional removal of infected nail material. This can significantly improve active ingredient penetration and support therapeutic success, especially with thickened nails.

Also important:

  • Pressure relief for painful, thickened nails
  • Advice on shoe and foot hygiene (to avoid reinfection)
  • Co-treatment of athlete's foot

3) Systemic therapy (tablets), for moderate to severe cases

For moderate to severe onychomycosis, the guideline recommends oral (systemic) therapy, provided there are no contraindications.

The guideline lists terbinafine, itraconazole, and fluconazole as systemic options; the selection depends, among other things, on the pathogen, comorbidities, interactions, and tolerability.

Safety aspect: Systemic antifungals can have interactions and are not suitable for everyone. This should be medically checked. It is best to consult your dermatologist.

4) Combination therapy, often the best strategy for extensive infections

The guideline emphasizes: For more severe infections, a combination of systemic and topical therapy should be pursued. This aims to improve mycological cure and reduce relapses.


🟢 Green : rather "mild" (usually possible without tablets)

  • 1 nail affected (or a few)
  • < 50% of the nail plate affected
  • Infection distal/lateral, not near the lunula/matrix
  • No pronounced nail thickening (no massive hyperkeratosis)
  • Patient can consistently apply lacquer/treatment

Typical procedure (Green)

  • Topical antifungal lacquer (e.g., Amorolfine/Ciclopirox) +
  • Regular podiatric nail reduction/debridement +
  • Co-treat athlete's foot (if present) +
  • Shoe/sock hygiene & reinfection protection


🟡 Yellow : "moderate" (combination urgently to be considered)

  • 1–3 nails affected or recurrent
  • ~ 50% involvement or increasing involvement
  • Infection begins proximally, but matrix not clearly/extensively involved
  • Nail thickening/crumbling nail hinders penetration
  • Risk of reinfection (Tinea pedis, heavy sweating, sports, tight shoes)

Typical procedure (Yellow)

  • Topical + consistent debridement as a basis
  • Early consideration of combination:
    • possibly systemic therapy (medical) or
    • Add-on procedures (laser/PDT/plasma) only in addition, not instead of basic therapy
  • Consistently co-treat foot skin & shoes
  • Follow-up after 8–12 weeks: if no clear response → escalation

 

🔴 Red : "severe/complicated" (usually tablets necessary, under medical supervision)

  • > 50% of the nail affected or
  • Matrix involvement / infection up to the lunula or
  • multiple nails (e.g., ≥ 3–4) or
  • Thick, severely dystrophic nails (massive hyperkeratosis/onychogryphosis) or
  • Immunosuppression, diabetes with complications, relevant circulatory disorder (PAD), frequent recurrences

Typical procedure (Red)

  • Systemic antifungals (medical; check interactions/risks) +
  • Topical therapy + debridement as a combination partner
  • Consistent Tinea pedis therapy + hygiene/shoes
  • Closer controls and possibly renewed pathogen detection in case of therapy failure

 

Step 3: Prevent relapses, so the nail stays healthy


Nail fungus has a significant relapse rate. The "last 20%" are crucial for success:

  • Consistently co-treat athlete's foot (skin is often the source of infection).
  • Keep feet dry, change socks, breathable shoes
  • Regularly dry/change shoes (for frequent athlete's foot, also hygienic measures)
  • Cut nails straight, not too short, avoid pressure points
  • For risk groups (e.g., diabetes), seek professional support early


Conclusion: Guideline-compliant treatment with patience and system


Nail fungus is treatable, but therapy requires:

  • Diagnosis confirmation (not "on suspicion"),
  • a severity-appropriate strategy (local, systemic, or combined),
  • and podiatric support to bring active ingredients to where they are needed.
